"Every system has basic venti- lation," says Mr. Bonnette, "and most patients in the outpatient setting are healthy. But sometimes you encounter sick lungs. Advanced modes can match the ventila- tion used during surgery to that used in the intensive care unit." Other settings may be better suited for pediatric and neonatal patients, he says, and the speed and pressure of gas delivery is a critical concern with trauma patients. Every provider needs to ensure the anesthesia machine is in proper working order at the beginning of every day and before each case. This has been made more automatic. A pre-use self-check is a standard feature in all workstations. "A provider will start the test, then come back later and fix what the machine says needs to be adjusted," says Mr. Bonnette. "But with some newer machines, you can set a timer to start before you arrive. It'll do the pre-use check and have an update ready by the time you arrive." While some calibration steps remain manually operated, "this saves a lot of effort," he says. Another effort-saving advance is a "pause button" for anesthesia, which offers a solution to the problem of what to do about the gas flow when a patient must be repositioned mid-procedure. Up until now, says Mr. Bonnette, you've had 2 options: Either do nothing, potentially letting a small amount of agent escape into the room, or turn all the knobs to zero, bringing anesthesia delivery to a halt. "Now there is a button that can push to put everything on hold," he says of the simple fix with significant time, labor and safety implications. OSM
