But by using a form I developed, we're able to keep everything straight. The individualized form, which we place in the front of each patient's chart and in a reference book we keep handy, makes it easy to see a wide swath of information: Does the doc use just 2 pre-op eye drops, or 4, or 5, or even 6? Does he send drops home with patients? Does he use pre-op medications like Xanax or Valium? What about eye patches? And how soon should the follow-up appointment be? It's all right there on the form. The form also lists both the generic and brand names of all the drops each surgeon uses, and includes a space for contraindications based on allergies. We update the forms every year or any time a physician makes changes. It works well and is something that can easily be adapted to a variety of specialists.
