Every member of the OR staff has to know exactly what's expected of them and when. • Egos need to be in check. Two of the greatest opportuni- ties for improvement for most facilities are likely to be improved efficiency and physician alignment — and the two are closely connected. To successfully run a same-day total joint program, physicians need to be willing to check their egos at the door, because everything needs to be scripted and everything needs to be standardized. Everyone should be involved in the planning. For example, to maximize efficiency, you may decide you need to be able to do everything with just 2 trays of instruments. That's something your reps will need to be able to provide. • Setting patient expectations is essential. You want to start discharge planning before the patient arrives. Have clear handouts for patients, so they know exactly what to expect. Be sure to tell them that you expect them to go home the same day.
